Abstract Background: Spinal cord injury (SCI) is a devastating neurological condition lacking effective regenerative therapies. Autologous bone marrow-derived mesenchymal stem cells (BM-MSC) have shown promise in preclinical and early clinical studies, but real-world clinical data remain limited. Methods: This retrospective case series analyzed 8 patients with complete thoracic SCI (ASIA A) who received autologous BM-MSC therapy at a medical center in central Taiwan between 2021 and 2024. The treatment regimen comprised bone marrow aspiration, ex vivo expansion of BM-MSC, repeated intrathecal administration via lumbar puncture (4–6 doses), combined with granulocyte colony-stimulating factor (GCSF) and riluzole as adjunctive therapy.
